    Registration of Persons Act amended Featured

    The Registration of Persons Act No. 32 of 1968 has been amended to provide a Common Application Form instead of the three forms currently in use, Commissioner General at the Department for Registration of Persons R. M. S. Sarath Kumara said.

    Accordingly, Form D. R. P V/01 which is used to apply for an identity card under Section 9 of the Registration of Persons Act, Form D. R. P. V/07 which is used to apply for a duplicate of a lost identity card under Section 16 of the Registration of Persons Act and Form D. R. P. V/08 which is used to apply for an amended duplicate under Section 17 of the Registration of Person's Act will be withdrawn on March 29, 2016.


    Commissioner General Sarath Kumara said new application forms can be obtained from any District Secretariat, Divisional Secretariat, Grama Niladhari, School Principals and Estate Superintendents. Additionally, he said, the form can be downloaded from the Department's website.

     

    The completed form must be certified the appropriate certifying officer - District Secretariat, Divisional Secretariat, Grama Niladhari, School Principals or Estate Superintendents and sent to the Department for the Registration of Persons together with all other relevant documentation and photographs.
